This newly createdAmbulatory Clinical Pharmacist embedded within Rheumatologyrepresentsa strategic investment in specialty care, patient access, andrelationship-basedpractice. This role is designed for a pharmacist who wants to practice at the top of their license, buildlong-termpatient relationships, and help shape how specialty pharmacy services integrate directly into outpatient clinical care.

The Role

This is anewly created,clinic embeddedambulatory clinical pharmacist positionsupporting UVA Healths rheumatology physician group. While the role is aligned withUVA Specialty Pharmacy, most of the work isclinic based, partnering closely with rheumatology providers, nurses, and patients to manage complex medication therapy across the continuum of care.

You will complement an existing rheumatology pharmacist and help expand services, capacity, and clinical impact as the program continues to grow.

Serve as acore clinical partnerto rheumatology physicians, advanced practice providers, nursing, and clinic staff.

  • Providelongitudinal ambulatory pharmaceutical care, supporting patients over time rather than episodic encounters.

  • Design, implement, assess, monitor, and documentevidencebasedtherapeutic plansfor patients with complex rheumatologic conditions.

  • Play a meaningful role inmedication access, including specialty pharmacy workflows, prior authorization support, and limiteddistribution ofdrug access.

  • Supportspecialty pharmacyaccreditation relatedprocessesthat directlyimpactpatient access to therapy.

  • Providepatient education, counseling, and ongoing medication monitoring, fostering trust and adherence.

  • Participate incollaborative practice agreements, including ordering labs and medications when applicable.

  • Potentially supportcomorbid condition management ordisease statecarewithin scope and training.

  • Document care plans and clinical interventions clearly within the medical record.

  • Rounding or coordinating with rheumatology providers and clinic teams.

  • Meetingpatients for medication education,follow-up, and therapy optimization.

  • Addressing medication access barriers and coordinating specialty pharmacy services.

  • Reviewing labs,monitoringtherapy effectiveness and safety, and adjusting plans under collaborative agreements.

  • Serving as the primary embedded pharmacist in clinic at times, working independently whileremainingdeeply connected to the broader pharmacy and specialty teams.

MINIMUM REQUIREMENTS:

Education: Bachelor's in Pharmacy or Pharm.D. from an ACPE accredited College of Pharmacy required.

Experience: No experience required. PGY-1 residency or equivalent clinical experience preferred.

Licensure: Virginia State Board of Pharmacy License or within 60 days of hire. American Heart Association (AHA) Health Care Provider BLS certification required.

PHYSICAL DEMANDS:

Job requires sitting for prolonged periods; Repetitive motion: typing. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull less than 20lbs. May be exposed to fumes, chemicals, blood/body fluids and infectious disease.

The starting base rate for this role is $64.37 hourly. Individual compensation will be determined by the selected candidate's qualifications, previous work experience, and/or education.
